Program exclusions and limitations
This dental discount plan is not an insurance plan that can be used at other dental offices. No refunds of premiums will be issued at any time if participants decide not to utilize the dental plan.

Terms and Conditions: Upon signing the contract for this in-office plan, you agree to make a full 12-month commitment. If the 12-month commitment is broken, any and all discounts received during the contract term will be retroactively applied to the account. The guarantor will be responsible for paying back any discounts provided if the contract is terminated before the 12-month term.
